ChristianaCare HomeHealth is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated team caring for primarily geriatric patients throughout the community. Position Highlights

• Full-time, day shift schedule

• One weekend per month

• Two holidays per year

• Limited on-call responsibilities

• Care for patients throughout the community

• Autonomy to manage your daily schedule while supported by an interdisciplinary team

Primary Responsibilities

• Provide skilled nursing care and prescribed treatments to predominantly geriatric patients in their homes in and around North Wilmington, DE

• Perform comprehensive patient assessments and ongoing evaluations

• Develop, implement, and update individualized care plans

• Administer medications, treatments, and IV therapy as ordered

• Educate patients and caregivers regarding disease management, medications, safety, and wellness

• Coordinate care with physicians, therapists, social workers, and other healthcare professionals

• Facilitate discharge planning and transitions of care

Qualifications

• Current RN licensure in Delaware or eligibility through a Compact State License

• BSN preferred; ADN candidates may be considered with a commitment to obtaining a BSN within the designated timeframe

• Minimum of one year of RN experience

• Home Health, Medical-Surgical, Geriatric, or Community Health experience preferred

• IV therapy and wound care experience preferred or willingness to complete training

• Current valid driver's license and reliable transportation

Hourly Pay Range: $36.10 - $57.76This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
