Join a relationship-based primary care team dedicated to serving medically and psychosocially complex adult patients at ChristianaCare's Thrive Practice!

The Thrive Practice is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive, patient-centered primary care in a collaborative, multidisciplinary environment.

Key Responsibilities

  • Deliver comprehensive primary care to adult patients with complex medical and behavioral health needs.
  • Develop and manage individualized care plans focused on patient goals and outcomes.
  • Coordinate care across healthcare settings and community resources.
  • Educate and counsel patients and families using evidence-based teaching strategies and teach-back methods.
  • Collaborate with physicians, care managers, and other healthcare professionals to optimize patient care.
  • Participate in quality improvement, population health, and value-based care initiatives.
  • Maintain accurate and timely documentation within the electronic health record (EHR).
  • Support onboarding and precepting of APCs, students, and other learners as needed.
  • Promote an exceptional patient experience through compassionate, relationship-based care.

Qualifications

  • Graduate of an accredited Nurse Practitioner (Adult, Adult-Gerontology, or Family Medicine) or Physician Assistant program.
  • Minimum 3 years of experience in primary care, internal medicine, family medicine, or complex care management.
  • Current Delaware licensure and board certification as:
  • Advanced Practice Registered Nurse (APRN), or Physician Assistant (PA-C)

Annual Compensation Range $112,765.00 - $147,511.00This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
