ChristianaCare is currently hiring a full-time Nurse Navigator II for the Outpatient Services at the Helen F. Graham Center located in Newark, DE.

Work Schedule:

  • Monday-Friday, day shift
  • No weekends or holidays required

PRIMARY FUNCTION:

The Oncology Nurse Navigator provides individual assistance to oncology patient populations and their families and caregivers to help overcome healthcare system barriers. While using the nursing process, the oncology nurse navigator provides education and resources to facilitate and coordinate care of identified patients with cancer and collaborates with the team of health care professionals to ensure timely, high-quality care across the cancer continuum.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Coordination of care of identified patients with cancer.

  • Identifies patients for referral to a multidisciplinary team or appropriate provider (surgery, radiation, medical oncology) through NCCN clinical guidelines.
  • Conducts initial standardized screening to identify psychosocial issues, distress, financial needs, fertility needs, barriers to care, and educational needs. Repeats screening at regular intervals or as needed.
  • Provides timely referrals and facilitates scheduling of appointments to appropriate support services, including but not limited to Oncology Social Work, Oncology Nutrition, Psychosocial Oncology, and other resources based on screening results and as needed. Monitors patient follow through.
  • Coordinates diagnostic procedures and specialty appointments, as necessary.
  • Contributes to the health care team’s development of the patient’s plan of care.
  • Documents all aspects of care coordination.
  • Facilities prevention, detection, and screening activities as appropriate. This may include nutrition, exercise, smoking cessation, and other wellness programs.
  • Attends Tumor Conference and supports data collection per Commission on Cancer Standards
  • Coordinates Multidisciplinary Center (MDC) visit care plan per provider direction.

Monitors patients’ progress according to plan of care; consults with other health care professionals as needed. Provides feedback to health care team at regular intervals and as necessary.

  • Collaborates with other programs, including but not limited to Cancer Research, Supportive & Palliative Care, Genetics Program, Community Health & Cancer Outreach, and Survivorship Nurse Navigator for patient referral.
  • Liaisons with referring physicians. Ensures delivery of communication according to the patient’s plan of care and progress
  • Collaborates with multidisciplinary team to support patients’ clinical treatment plan.

Ensures that patient, family, caregivers are knowledgeable of the plan of care throughout the cancer care continuum.

  • Assesses patient and family learning needs to identify barriers to education (e.g., literacy, language, cultural influences, comorbidities)
  • Utilizes “Teach Back” method for patient education.
  • Educates the patient/family/caregiver: diagnosis, treatment options, side effect management, post treatment and survivorship.
  • Assists patient/family with appropriate information to make an informed decision.
  • Monitors the patient family participation in the plan of care, empowering patients to be active in self care management and shared decision making.
  • Provide education regarding the plan of care; answer questions and/or refer to appropriate resources.
  • Advocates for patient and family
  • Provides emotional support for patient and family and/or identifies internal and community resources for same.
  • Facilitates communication among patients, family members and health care team.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • BSN required; MSN preferred.
  • Two or more years of combined clinical experience in oncology care or case management preferred.
  • Oncology Certification (OCN) preferred or the completion of Oncology-specific contact hours per Commission of Cancer Standard requirements.
  • Delaware RN licensure required. Pennsylvania, New Jersey, and Maryland licensure may be necessary.
  • BLS certification required.
Hourly Pay Range: $41.28 - $66.05This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
