ChristianaCare Wilmington Thrive Primary Care is searching for a Social Worker II.

Wilmington Thrive Primary Care is located on the Wilmington campus. The address is 501 West 14th Street, 4th Floor of the Gateway Building in Wilmington, Delaware. Wilmington Thrive Primary Care is slated to open this upcoming November 2025.

The practice will consist of one full-time Primary Care physician, one part-time Cardiologist, a registered nurse, and a medical assistant. As the practice expands and grows, additional caregivers will be allocated.

The Social Worker II will aim at assisting patients and families in identifying their health, social, emotional, and environmental needs. The role involves connecting these individuals with available resources and services by providing a comprehensive range of longitudinal health and social work services.

Work Schedule:

  • Monday through Friday from 8am to 5pm

Duties / Responsibilities:
As a Social Worker II, you will focus on addressing the social care needs of patients by identifying their needs, supports, barriers, and community resources. Proficiency in electronic systems, including Unite Delaware and electronic medical records, is essential for providing information on social service programs and regulations at both state and federal levels. Responsibilities encompass conducting screenings related to social determinants of health and formulating plans to address identified needs. Organizes, integrates, and modifies essential resources through collaboration with patients, families, and various agencies to ensure comprehensive support and promote self-management of social needs. Be involved in gathering detailed information regarding patients' social circumstances, offering insights on available community resources, and coordinating the delivery of services. Interpreting and educating individuals on resource availability, maintaining timely documentation, and actively participating in departmental and multidisciplinary meetings. Furthermore, the position supports educational initiatives by supervising students and community health workers.

Education Requirements:

  • Master’s degree in social work (MSW) from an accredited graduate school of Social Work.

  • One to two years of direct experience with arranging community services for complex populations preferred.

  • Minimum of two years of related work experience preferred.

  • Community visits as needed.

  • Bilingual language skills are a plus.


Strengths and Qualities:

  • Knowledge and skills in social work include crisis intervention, human behavior theories, community resources, and access to state and federal programs. Key abilities include teamwork within multidisciplinary teams, organizing work assignments, evaluating patient needs and care plans, and assessing the effectiveness of services. Familiarity with medical terminology, flexibility in dealing with complex patients, and timely documentation are essential. Additionally, an understanding of biopsychosocial assessment tools is crucial.

Hourly Pay Range: $30.34 - $48.55This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
