Do you have a strong interest in public health and the drive to guide a diverse team? Join Christiana Care as a Community Health Program Coordinator where you will be instrumental in steering our Women's Health Community Health Worker Program.
The ideal candidate should possess experience in program evaluation and a deep understanding of the importance of addressing inequalities in women's healthcare.
Responsible for the day-to-day administration and management of Community Health programming, specifically Community Health Workers groups
Recruit and supervise Community Health Workers – interviewing, hiring, training and all day-to-day oversight requirements.
Successfully work closely with internal and external partners to plan, facilitate, and support programs.
Assure that program goals are met, and data is collected by staff as required.
Assure timely and accurate data reporting as required.
Support project evaluation initiatives.
Facilitate team meetings and support groups for patients, as needed with CHW team.
Attend all required internal and external meetings, acts as representative on appropriate committees.
Attend trainings and meetings as scheduled and required.
Educate staff, health providers and stakeholders about community needs.
Advocate for underserved individuals to receive appropriate services. Works to reduce cultural and socio-economic barriers between clients and institutions.
Collect data as required by supervisor.
High School diploma required; bachelor’s degree preferred.
Two years’ experience in program administration, program leadership, coordination, etc. required
Prior experience working as a Community Health Worker is helpful, but not required.
Proven experience in people leadership is a plus.
An equivalent combination of education and experience will be considered.
Preferred skills include public speaking, group facilitation, and excellent writing.
Computer proficiency is required.
