ChristianaCare HomeHealth is seeking a Full Time LPN Nurse Liaison. The successful candidate will support New Castle County, DE working out of our North Wilmington Office; work day shift hours 8am-4:30pm, and every other weekend day, some holidays may be required based upon departmental needs.

Duties and Responsibilities:

• Develop Relationships with referral sources, providers and other health care professionals.
• Pre-admission evaluation and education of patients identified for home health to identify care needs, available resources, and validate information needed to ensure safe transfers across the care continuum.
• Perform ICD-10 coding of referrals.
• Coordinate with branch office manager to schedule start of care and supplies for services ordered by the provider as well as communication with referral sources and providers.
• Identify patients appropriate for specialty programs and telehealth.

Requirements:

• LPN
• Previous case management or discharge planning preferred.
