Under the direct supervision of the Clinical Director, the Clinical Manager provides oversight for the care rendered to the patients and caregivers on their team. The Manager is responsible for the supervision and management of the professionals assigned to their team. This position is responsible for the delivery of exceptional quality/patient outcomes and for alignment with branch budget. Specific territory that we are seeking coverage for is a subset of New Castle County focusing on the Newark and Glasgow areas. This person will work in the office located on Avenue North, in North Wilmington, DE as well as spend time in the field with their staff and be responsible for committing to rotations for weekend, holiday, and on-call rotation.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Demonstrates knowledge and skills necessary to provide care to adolescent, adult and geriatric patients, including the ability to obtain and interpret information to identify patient needs.
- Assures compliance with regulatory and professional standards.
- Identifies priorities of management responsibilities and demonstrates the ability to delegate.
- Develops and maintains method to ensure patient satisfaction for assigned team.
- Performs routine co-visits, clinical record reviews, case conferences and patient rounds to establish visibility, teamwork, and monitoring the level of care/practice.
- Remains cognizant of risk management issues, and takes appropriate measures to correct.
- Enables a relationship based care approach to clinical practice and care delivery in interprofessional team, including utilization of operational reporting to drive accountability.
- Promotes the autonomy of the clinical staff, encouraging independent decision making and problem solving.
- Supports and maintains shared decision-making for team and a culture of nursing professional governance.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- RN Licensure accepted in the State of Delaware.
- Completedf BSN Degree in Nursing Required; Master's degree nursing rquired or a commitment to complete degree within 3 years of starting position.
- OASIS Certification required; or willing to become certified within 2 years.
- Nurse Executive Certification preferred.
- Basic Life Support (BLS) required.
- Three (3) years of management experience or demonstrated progressive leadership development.
- One (1) or two (2) years of Home Health and Hospice or community care experience preferred.
Proof of a valid driver’s license and active auto insurance is required.
