Swank Center for Memory Care and Geriatric Consultation is located at 501 West 14th Street, Wilmington, DE 19801. The Swank Center for Memory Care and Geriatric Consultation also has locations in Smyrna and Rehoboth, Delaware. Swank Center for Memory Care and Geriatric Consultation has been selected as a GUIDE model by the Centers for Medicare & Medicaid Services (CMS), enhancing its ability to provide comprehensive dementia treatment plans and provide necessary resources to caregivers.
As the Social Worker, you will join the Guide team in supporting the Community Educator, conducting home visits, and assisting patients and families in identifying social, emotional, and environmental needs. Provide social work services, including case management for patients in medical social services programs, including geriatric and disabled patients. This role aims to support an innovative healthcare model.

Job Duties / Responsibilities:
As the Social Worker, you will be providing direct casework services, including emotional and social assessments, problem identification, and developing a service plan to assist patients and families in dealing with medical conditions. You will assess financial eligibility of families seeking financial, medical, or related services and provides referrals to alternative sources. You will be involved in individual and family counseling, coordinating public, private, or community medical, social, and therapy services. You will review patients' case history, evaluates their potential for further treatments, and prepares recommendations and referrals. It also prepares social histories, patient progress reports, evaluations, and summaries. Provide medical social work consultations, processes referrals, manages clinician waitlists, and completes patient safety assessments and intake. Actively participates in department and team meetings, representing the GUIDE Team at system and external levels. Ensures safety by adhering to departmental safety rules and practices, reporting unsafe activities, hazards, or safety violations to appropriate personnel. Demonstrates skills and knowledge necessary for providing care appropriate to adult and geriatric patients, including knowledge of growth and development, obtaining, and interpreting information, and providing necessary care.
